About Us
With Grace Hospice and Palliative Care is a locally established hospice organization, founded in Santa Clara County eight years ago. Our services are designed to cater to the unique needs of our patients, and we provide care in the comfort of their own homes. As a faith-based company, we strive to provide compassionate and respectful care that supports the spiritual and emotional well-being of our patients and their families.
Our commitment to excellence is reflected in our accreditation with the Accreditation Commission for Health Care (ACHC). This achievement demonstrates our adherence to the highest standards of quality care in the industry.
We take pride in giving back to our local community and beyond by contributing a portion of our profits to support charitable causes. As a team, we are committed to supporting our patients and their families in every way possible. We leverage the unique gifts and talents of our team members to deliver holistic care that meets the physical, emotional, and spiritual needs of our patients.
