Our hospice care includes visits from nurses, social workers, chaplains, home health aides, and volunteers to the patient’s home, 24-hour on-call nursing support, medication for pain and symptom management, durable medical equipment, counseling, spiritual support, respite care for caregivers, and anticipatory grief support for family and loved ones.

Our mission is to offer hospice services to all terminally-ill patients, regardless of payment ability. Care is covered by Medicare, most insurance plans, and managed care organizations, with no billing to patients or families.

About Us

With Grace Hospice and Palliative Care is a locally established hospice organization, founded in Santa Clara County eight years ago. Our services are designed to cater to the unique needs of our patients, and we provide care in the comfort of their own homes. As a faith-based company, we strive to provide compassionate and respectful care that supports the spiritual and emotional well-being of our patients and their families.

How we can help

Our belief is that everyone deserves a comfortable end of life experience. Hospice services are covered by Medicare, MediCal, or private insurance. For those unable to pay, With Grace offers core services at no cost. No bills will be received for hospice services.

Nurses

Experienced nurses are available 24/7 as a resource or to address any medical concerns to assure proper care and peace of mind.

Medication

Hospice provides all medications that are related to comfort and symptom management. No need to worry about calling the physician for refills or seeing the doctor when you run out of medications.

Hospice Aides

Helpful aides assist with personal care, such bathing and dressing, that can be difficult for families to do alone.

Volunteers

Caring people provide companionship and support such as running errands, shopping, reading, and short term relief for caregivers.

Social Workers

Knowledgeable people provide resources that are available in the community and can assist with health/social applications, planning, and counseling.

Spiritual Counselors

Compassionate people provide emotional and spiritual support to the patient and their families, including prayer.

Our founders, Dr. Marvin Masada and Linh Le, RN, created our agency with a desire to offer compassionate and professional hospice care. We view our work as a service to God, and approach it with joy and mercy instead of obligation. We value our relationships with patients and their families, and go beyond just doing our jobs.

What is hospice care?

When is the time to consider hospice care?
It may be time to consider hospice care for your loved one if they require more assistance with daily needs, experience a loss of awareness of surroundings, have significant weight loss, incontinence of bladder or bowels, recurrent infections, or pressure ulcers. If you’re experiencing any of these signs, please make an appointment with your Primary Care Physician to discuss your care needs and request support.
What is the cost of hospice care?

Hospice care is generally covered by Medicare, most insurance plans, and other managed care organizations. At our agency, we believe that everyone should have access to hospice services, regardless of their ability to pay. Therefore, we never bill hospice services to the patient or their family, regardless of their insurance status.

What is Palliative care?
Palliative care is specialized medical care for people living with a serious illness. The focus is on providing relief from the symptoms and stress of the illness, with the goal of improving quality of life for both the patient and their family. It can be provided at any stage of illness and can be provided alongside curative treatment.
How do I get hospice care?
To get hospice care, ask your physician to fax an order for hospice along with the History & Physical to your preferred provider. Share your insurance information, and we’ll contact you to schedule admission after reviewing the documents. The process time can vary.
How can a loved one be eligible for hospice care?
To be eligible for hospice, a patient must have a life-limiting or terminal illness and be certified by their primary care physician with a life expectancy of six months or less. The patient should also have completed all curative therapies or treatments.
